Request a Lab or EKG
Welcome to WF Paramedical Services. We are a CLIA Certified Mobile Lab providing CLIA waived tests, including Rapid COVID-19, Flu A and B, Strep A, Lipid Panel with Glucose and PT/INR. We also provide specimen collection services for your patients, process the specimen and deliver it to your designated lab. We work closely with Clinical Reference Laboratory if you are in need of a lab based test at a CLIA Certified Facility. Additionally we offer in home EKGs and provide interpretive results same day.
Lab/EKG request can be submitted on the form below. Requests that contain protected information can be faxed to our office at 866-538-7337. Once received, we will contact your patient to set the earliest possible appointment and send one of our experienced professionals to complete the request.
